Because we depend solely on volunteers and we do not have coverage in all areas of the country, it is possible that your adoption process and approval may be delayed or even be geographically impossible. The more active volunteers we have, the more efficient our ability to respond to the needs of the dogs and the adopters will be. Please consider volunteering. The Contract & Liability Release forms cannot be Emailed, you will be instructed to hand them to the representative conducting the home visit. Applications are submitted electronically.
I/We hereby make application to the AMERICAN LHASA APSO CLUB RESCUE. I/We agree to have my/our name(s) added to the waiting list for the purpose of giving a Lhasa Apso a permanent home., and understand that when I/We receive the Lhasa it will have:
1. Been spayed or neutered
2. Had a thorough checkup from a licensed veterinarian, including current shots.
3. Had any necessary health care treatment(s) as determined by AMERICAN LHASA APSO CLUB RESCUE
I/We understand that the Lhasa may not be sold or otherwise given by myself/ourselves to any other person or organization without the written permission from AMERICAN LHASA APSO CLUB RESCUE.
I/We will have an identification tag on the animal at all times with our current address & phone number. Further, by giving the Lhasa Apso a permanent home, I/We accept full responsibility for the care of the Lhasa and agree to:
a) In the event I/We adopt a Lhasa and for any reason can no longer care for or keep the Lhasa, we agree to immediately RETURN the Lhasa to AMERICAN LHASA APSO CLUB RESCUE. If this occurs, I/We will sign the necessary papers for the surrender of the Lhasa.
b) Notify AMERICAN LHASA APSO CLUB RESCUE within 24 hours in case the Lhasa is lost or stolen and make every effort to locate the Lhasa.
c) Never take this Lhasa to an animal shelter for the purpose of surrender.
d) Never abuse, hit, or mistreat this animal.
e) Properly house, feed, water and care for this Lhasa.
f) Exercise proper control and restraint over this Lhasa at all times and not permit this dog to run at large.
g) Comply with all State and Local ordinances relating to the keeping of this dog, including leash and license laws.
h) Not use this animal for any type of experimental or breeding purpose.
i) Provide regular veterinary care, including annual vaccinations, parasite checks, dental care and regular grooming.
j) Provide exercise, adequate companionship and love.
k) **Will not leave dog alone in fenced yard unless gate has been locked. Will not stake or tie the dog outside unattended. (We hear of instances where pets are taken out of fenced yards or stolen off of tie-outs while their owners are home! Many small breeds are used for training Pitbulls to kill or in Occult killings).
l) Ensure this Lhasa is maintained predominantly as an inside dog, as Lhasas are not equipped to live outside all the time. I/We agree to keep the dog inside the house when no one is at home, and agree that the dog will sleep inside the house at night.
m) Agree that should we move residence, whether in state, out-of-state, or out of country, we will keep AMERICAN LHASA APSO CLUB RESCUE informed of our current address. If we must move out of the area and we find we are for any reason unable to keep this Lhasa, we will immediately contact AMERICAN LHASA APSO CLUB RESCUE so we can be referred to a local rescue group associated with national Lhasa Rescue and work with said group to find an appropriate home for the Lhasa.
n) Never abandon this Lhasa Apso
o) To allow representatives of AMERICAN LHASA APSO CLUB RESCUE to periodically check on the Lhasa to assure compliance with these items listed (a through n).
I/We have read the foregoing and agree with all the provisions and stipulations stated herein. The AMERICAN LHASA APSO CLUB RESCUE reserves the right to refuse any applicant. If the above statements are found to be fraudulent or untrue: WE RESERVE THE RIGHT TO REPOSSESS THE LHASA AND ALL LIABILITIES ARE WAIVED. THIS IS A BINDING CONTRACT, ENFORCEABLE BY CIVIL LAW. ADOPTION APPLICANT(S): Must be signed by ALL ADULTS in household responsible for this Lhasa.
